Against limits: „Three-dimensional reconstruction of a bone defect with an immediate implantation.”
Introduction
The aim was a reconstruction of the front incisor by the use of a dental implant. In addition, the patient suffered from a significant bone defect in vertical and transverse diameter dimension. The surgical part was reduced into a single procedure by the vertical three-dimensional augmentation of bony defects and implantation.
Method
Dental implant Ankylos A11, Bio-Oss bone substitute, Bio-Gide membrane with pins, Zepf 3D surgical kit. To achive a 3D bone regenration “The RingGraft method” by Dr. Bernard Giesenhagen was used. The RingGraft was harvested from the chin by the 8mm trepan. The jaw was prepared for the graft by the 7mm trepan. The basis was stabilized by the dental implant. Small bony defects were supplemented with the bone shavings and Bio-Oss. The entire area was covered by blood clot and Bio-Gide.
Results
After six months the dental implant was exposed. There was a slight resorption of cortical bone from the palatal side. The last step was a prosthetic restoration on BalanceAnterior abutment with keeping papillas and gingival margin.
Discussion
The patient’s restricted time limit caused choosing the above mentioned treatment method, which reduced the surgical part into a single procedure. A small RingGraft defect did not influence the prosthetic stage. The cause of the RingGraft defect might have been a difference in bone structure of graft which had more cortical bone at palatal side. The another factor might have been a bad hygiene and no control visits within time of osseoinegration
Conclusion
Time reducing technique – reducing the surgical part into a single unit is a very good solution for patients with time limitations, like sailors, travellers, drilling rig workers, etc. as well as for the patient with fear and phobias. Nevertheless, the above mentioned method carries the risk of the loss of part of the RingGraft.
